Endermologie TREATMENT PROGRAM Contract

At the commencement of a course of Endermologie you will be asked to enter in to a contact, where you are obligated to sign and date it agreeing that you understand the conditions set out to achieve the best desired results and it reaffirms your level of committment to the program.  This contract is a partnership and we are committed to deliver the best possible results on your behalf and we will work with you to acheive this -

You must understand that in committing to a treatment program, you agree to attend the appointments scheduled that you have elected to achieve the best possible results.   

You must understand the treatment plan results dependant upon following the recommended exercise programme that best suits you and you must drink 3 litres of water prescribed per day in partnership with Endermologie treatments.

You must be aware of the appointment cancellation policy at In Form Clinic, if you do not give more than 24 hours notice cancellation you will be charged 50% of the treatment cost and if you do not keep your appointment with out notification to us you will be changed the full cost of the treatment.

If you wish to cancel this program before the agreed course is completed you understand you will be charged full price $150 for each treatment redeemed at the time of cancellation. 

You understand the treatment program can be put on hold for a limited period of six months and you are able to redeem the treatments still outstanding at that time. 

You must understand that you will not be refunded on treatments not redeemed should you choose to cancel this program at any time for any reason.   

You will be asked to sign a hard copy of this contact that you have read and are in agreement to the conditions set out.

We strongly recommend you keep to the treatment schedule set by the endermologist to achieve the results desired, should you miss appointments, not follow an exercise plan or fail to drink the recommended daily water intake you run the risk of not achieving the desired results.
